Clear notes separate new content, adjustments and fixes. A change to a control should explain the behaviour players will now encounter. A repaired quest should identify the affected situation without revealing unnecessary story details. Broad phrases such as improved stability may be accurate, but they offer little help to someone tracking a particular problem.
Known issues deserve their own place. Acknowledging a remaining limitation gives players a way to distinguish an unresolved problem from an unexpected one. Where a workaround exists, it should be specific enough to follow and easy to find.
The tone matters as well. Patch notes are a continuing record of the relationship between a game and its audience. They work best when they are direct about what is available, careful about what is promised and consistent from one release to the next. A concise explanation can build more confidence than a long announcement that leaves the practical questions unanswered.
